The next training classes for both the Orthopaedic and Neurologic Residency programs will begin in January 2009.  The application deadline for both programs is August 31st, 2008.  Applications from external candidates are welcome at any time up until the application deadline. 

In order to apply to the Brooks Physical Therapy Residency Program, you will need to provide the following documents.

Admissions Checklist (download here)

  1. Application for Admission
  2. Copy of PT License or description of how to verify PT license (download form here)
  3. CPR Certification
  4. Immunization Record (including Hepatitis B, tetanus, etc)
  5. Tuberculosis Screen
  6. Three (3) Professional Recommendations (download form here)
  7. List of Continuing Education Courses attended in the last five years (download CEU Verification Form here)
  8. Personal Letter for Admission (download guidelines here)
  9. Resumé or CV
  10. Transcripts from PT School
  11. Transcripts from other post-professional degree programs
  12. Proof of health insurance (if Brooks benefit is waived)
  13. Resident/Fellow Contact Information Sheet

In addition to this material, you will also be asked to complete a Certification of Truth. This document certifies that you have not omitted or falsified any information and/or documents submitted within your application packet. This document requires signatures from yourself and a witness and must also be notarized. Admissions Committee

The admissions process for the Brooks Physical Therapy Residency Program is competitive, and a committee has been established to choose the residents from among the applicants. The committee includes:
